Waiver for Chicago Debates Volunteer Application
Release, Consent and License Agreement
Chicago Debates participates in, hosts, sponsors, organizes, facilitates and provides student debate
programming, including debate practices, tournaments, camps and other related events, and often records
those programs and the related debate presentations (collectively, the “Debates”) for use thereafter. In
consideration for the intangible value gained by participating in such an event, and for other good and
valuable consideration, the receipt and sufficiency of which are hereby acknowledged, I, the
undersigned, on behalf of myself as participant or as parent or official legal guardian of the below-
identified minor participant, (the “Participant”), hereby agree as follows:
I consent to and grant permission to Chicago Debates and its licensees, successors, assigns, and designees
(the “Licensed Parties”), to record, film, photograph, tape and otherwise capture and reproduce in any
manner the Participant’s name, voice, likeness and performance. Any such recording will hereafter be
referred to as the “Recordings.”
I grant the Licensed Parties (1) a non-exclusive perpetual worldwide right and license to use, edit, adapt,
modify, transcribe, publish, reproduce, exhibit, perform, create derivative works from, distribute, display,
license, or otherwise use the Recordings (and all rights connected therewith) in all forms and media,
whether now known or later developed, throughout the world; and (2) the non-exclusive perpetual,
worldwide right and license to use the Participant’s name, photograph, voice, likeness, any biographical
information provided to Chicago Debates, and comments made by the Participant in connection with the
Recordings. Such use may be for public relations, educational, or for any other purpose in support of
Chicago Debates’ mission.
I recognize and agree that Chicago Debates will be the copyright owner of the Recordings and all works
created in connection with the Recordings and that neither I nor the Participant have any rights with
respect to the Recordings, including, without limitation, rights of approval or moral rights. All negatives,
positives, prints, digital reproductions, and video recordings shall be the property of Chicago Debates.
I understand and agree that (a) the Licensed Parties may, but are not obligated to, use, edit, modify,
transcribe, publish, reproduce, exhibit, perform, create derivative works from, distribute, display, license,
or otherwise use the Recordings in any and all media now or hereafter developed for the purposes stated
above; (b) I will not receive any compensation for the Licensed Parties’ use of the Recordings; and (c) I
have no copyright or other rights with respect to the Recordings. I confirm that I have the authority to
grant this consent, release and nonexclusive license.
I acknowledge and agree that the Debates may be open to the media and on the record and may be
photographed or recorded by members of the media. In the event members of the media do attend, any
such media will be considered Licensed Parties as Chicago Debates’ designees.

This release (a) contains the entire understanding between Chicago Debates and me and the Participant
with respect to the subject matter hereof, supersedes all prior agreements with respect to the subject
matter hereof, and may not be altered or waived except by a writing signed by Chicago Debates and me;
and (b) will be governed by the laws of the State of Illinois applicable to contracts executed and to be
fully performed therein, without regard to its principles of choice of laws. The exclusive venue and
jurisdiction for any legal action arising out or relating to this release are courts of competent jurisdiction
located in the State of Illinois.
I and the Participant, on behalf of Participant and Participant’s heirs, assigns, legal representatives and
next of kin, release, waive, discharge and hold harmless Chicago Debates and its members, trustees,
agents, officers, contractors, volunteers, and employees from and against any and all claims, demands,
actions, complaints, liabilities, damages, costs, expenses or other forms of liability arising out of or
related to Participant’s attendance at or participation in the Debate(s), including the Licensed Parties’ use
of the Recordings or any portion thereof.
BY SIGNING THIS RELEASE, I ACKNOWLEDGE THAT I HAVE COMPLETELY READ AND
FULLY UNDERSTAND THE ABOVE TERMS AND AGREE TO BE BOUND THEREBY. I
HEREBY RELEASE ANY AND ALL CLAIMS AGAINST CHICAGO DEBATES AND THE
LICENSED PARTIES OR ANY PERSON OR ORGANIZATION UTILIZING THE PARTICIPANT’S
IDENTITY OR THE RECORDINGS
